Yes, you need to print the approval page with the amount you were approved for.  Take this printed page with you the next time you see your Cosmetic Dentist.

Do I need to take the printed page with my approved amount to my doctor’s office?

Do I need to know how much my procedure will be before applying?

Yes and no.  It helps to know how much the procedure will cost before applying for the loan.  This way you will borrow the correct amount, not too much or too little.  However, it is not necessary.  You may borrow whatever amount you prefer as long as you reach the minimum loan amount and do not exceed the maximum loan amount.
